Same at hospitals — the waiting room is public space so keeping your hat on is fine, but manners say you take it off once you step into the exam room.

I've worn a hat in restaurants for over 20 years and not once has anyone called it a manners violation — not staff, not other customers.
Plenty of other customers wear hats too, and I've never seen anyone act like it bothered them.
