Attack on Titan is way worse — episode 1 alone has the mom getting eaten by a giant humanoid creature, the kid's hometown gets overrun so he can't live there anymore, and he ends up a child soldier because he's got no family left. That's messed up too.

There's plenty of depressing or dark manga out there, but I can't think of anything more genuinely evil than Mii-yama.

There's kind of a growing trend in society where pointing out blunt, harsh truths is seen as tactless.

Even if that was the intent, I think the main issue is the author is too ignorant for it to actually read as realism.
Once it creates an echo chamber where ignorant people hype each other up and that gets broadly promoted, that's when real trouble starts.

Even though 'Ushijima the Loan Shark' is the same kind of shock-value manga, it was built on real research to get to that level.
