Once the defense has already had 3 chances to record an out in an inning, any further runs allowed aren't charged as earned. For example: with no outs, an infielder boots a routine grounder, letting the batter reach base instead of being retired. If the defense then actually records two outs on the following batters, it's treated as if the defense had gotten 3 outs total (i.e., the inning would have ended without the error). Any runs the offense scores after that point aren't counted as earned runs against the pitcher.
